I'm not certain where this term originated, but in the past few months I've seen it sprinkled about the web.
As near as I can determine, it refers to the loss of a (virtual) "fix" on your position, whether in cyberspace at large or on one particular web site. It often happens to unwary visitors after a drastic site redesign.
