The n-methyl groups covalently modify cellular DNA. The precise mechanisms by which this drug acts to kill tumour cells is not completely understood.

Lomustine is readily absorbed through the intestinal tract, it has a serum half-life of 72 hours.

Principle side effects are, nausea and vomiting. Thrombocytopaenia and leucopaenia can occur after 4-6 weeks due to the prolonged marrow toxicity of the drug.
