In essence, institutions (whether governmental, corporate, or otherwise), like individuals, can be complicit in certain actions or outcomes, either intentionally (wittingly) or unintentionally (unwittingly). It’s like when one "accidentally" leaves one's umbrella at home on a rainy day - the intent wasn't to get wet, but the forgetfulness contributed to the outcome.

Done with or in awareness or consciousness; performed with possession of the full (or at least the purported predominance of) facts of a situation; or, as defined by the Department of Defense's March, 2017, edition of the Dictionary of Military and Associated Terms, page 253:
A term of intelligence art that indicates that one is not only aware of a fact or piece of information but also aware of its connection to intelligence activities. (JP 2-01.2)
