In communist countries (particularly those in Eastern Europe), the first secretary was the leader of the communist party. Because of the nature of the one-party system, the first secretary also got to lead the government. Frequently "one-party system" was a code word for "dictatorship by the party" which was often a code word for "dictatorship by the first secretary" which is just a plain dictatorship.
