A mistake. Usually not a very bad one. Just a minor mishap. Can often be heard by mothers and other people who are prone to using less offensive language, although "goof-up" can sound fine in a sentence spoken by most anyone.
"What? 6 times 4 doesn't equal 28? Oh yeah... That was a goof-up."
