It either originally referred to an uncut diamond, or the vision of a diamond surrounded by regular rock. This phrase is used metaphorically either to describe someone or something that has untapped potential, or to describe someone or something that sticks out, something that shines above and beyond everything else around it. Kind of like the pearls before swine thing.
