Picard learns Spock's mission to Romulus involves a plan to reunify its people with the Vulcans.

Lieutenant Worf is paralyzed when several cargo containers land on him, and he opts to follow Klingon custom and commit suicide. This is one of the few episodes in which we see the Worf/Troi Relationship that was so often hinted at - he asks her to take custody of his son Alexander in his absence. Meanwhile, a doctor has come aboard who performs rather radical experiments on her patients. Coincidentally, this doctor (Dr. Russel) is Worf's only hope for walking again - she has the power to make a new spine for him. Dr. Crusher despises Russel's methods, but since Worf would rather die than not walk again, she allows the surgery. At the end of the surgery, Worf appears to die, but miraculously recovers minutes later as his backup organs kick in.

Comparable to Rejoined (DS9), this episode can be construed as either an analogy for transsexuality or for gay people. A member of an androgynous race secretly identifies as female and falls in love with Riker. Ey gives a very moving speech on how just because ey identifies as female, that doesn't make em sick or wrong in any way, before the authorities on eir planet brainwash em (reminiscent of the electric shock therapy used to try and "cure" transsexuals in the past) into thinking that ey was wrong to have these feelings and desires in the first place. This is a very emotional episode for anyone who's been in a similar position with their gender identity.
