Originates from a teacher's stereotypical remark on your report card. Its a euphemism for the cold, hard truth that it is possible that their kid might be suffering from a personality disorder of some kind. Some that spring to mind are: borderline personality disorder or sociopathological personality disorder or perhaps the kid is just more introverted than normal or perhaps its because of a lack of confidence. Who knows? Anyway, I got this remark so many times through high school it was funny. And look how well I turned out :)
